Business Name Change
- You are an existing business owner and are changing the name of your business.
- Upon submission of all required documentation, Suddenlink will change the name on your account, and, where applicable, make the necessary changes to your Directory Listing and Caller ID description as specified on the form.
Business Service Takeover
- You are a new individual or group of individuals and have acquired the ownership of a business and its operations, including responsibility for the Suddenlink account and equipment.
- Upon submission of all required documentation, Suddenlink will change the contact information on the account to the new owner, and, where applicable, make the necessary changes to your Directory Listing and Caller ID description as specified on the form.
Important: Direct Debit and Recurring Credit Card Payment Options must be canceled prior to submission of completed Business Name Change / Service Takeover Form
